Requesting Services
Students requesting accommodations, academic adjustments, and/or auxiliary aids must provide written documentation to the Disability Services office. Services cannot be provided for a student who does not provide the necessary documentation.
All requests for accommodations should be made at least 30 days in advance of the need. Once documentation is submitted, it will be reviewed in the order it is received, and all accommodations are determined on a case-by-case basis. Therefore, all requests should be made as early as possible.
Documentation must be written by a licensed or credentialed examiner; which includes a diagnostic statement identifying the disability; a description of the diagnostic methodology; a description of the current functional limitations; a description of the expected duration and progression of the condition; and a rationale and justification for all requested accommodations.
Appropriate documentation may include a relevant psychological evaluation; medical letter/report from a qualified evaluator. Along with a comprehensive evaluation, other helpful documents include an Individual Education Plan (IEP) and 504 Plan. Any and all information received by the college regarding an individual's disability is strictly confidential.
